Eosinophilic esophagitis (EoE) is a disease of the immune system that results in chronic inflammation of the esophagus. This inflammation is caused by a larger than normal presence of cells in the esophageal lining called eosinophils. This chronic state of inflammation can lead to swelling of the esophagus, which can cause a variety of presenting symptoms depending on the age of the person. Most often, it can present as reduced appetite, stomach upset and difficulty swallowing.
EoE is correlated with having other allergy-related conditions, such as food and/or environmental allergies, meaning that when a person with EoE is exposed to certain foods and/or environmental allergens, their symptoms worsen. Preexisting autoimmune disorders, like inflammatory bowel disease (IBD), can increase the likelihood of developing EoE as well.
Determining which specific foods trigger EoE is difficult, as the food allergies associated with the condition do not present the same way as a typical food allergy. This makes it hard to figure out what specific foods are causing the inflammatory response, and so it can take longer to determine individual triggers. The good news is that once food allergens are identified and removed from one’s diet, symptoms improve relatively quickly.
For this reason it’s imperative to seek the support of a dietitian for EoE.
How can a dietitian and nutritionist help manage Eosinophilic Esophagitis (EoE)?

- Cases like described above are where a Registered Dietitian (RD) can become a valuable tool. One of the primary treatments for EoE is some version of an elimination diet, which can involve cutting out all or some of the following foods: dairy, soy, wheat, eggs, peanuts, tree nuts, seafood and shellfish. Working with a dietitian through the elimination and reintroduction phase of this EoE treatment option can make it much easier to adhere to.
- In severe cases, people with EoE may require a strict elemental diet, which involves getting nutrition and fluid from protein-derived formula in order to avoid all possible allergens.
- In conjunction with dietary treatment, medicinal treatment and/or esophageal dilation may be warranted as well, and so it is very important to work closely with your medical team throughout this process.
