Are you looking to book an appointment with a nutritionist or dietitian? Here’s how it works:
Simply get in touch with us, and we’ll let you know what appointment time slots are available. Don’t forget to specify:
We then book an appointment for the initial session.
After booking an appointment for the initial session, we will send you an intake form. The form is designed to give our nutritionists and dietitians a clearer, more in-depth picture of your health concerns and goals. This allows us to serve you better.
We ask you to fill out the form and return it digitally prior to the initial session.
During the first appointment (i.e. initial session) we will co-create a personalized meal plan (not applicable to Singe Session, No Meal Plan) based on your intake form and input.
A few days after the initial session session, you will receive the personalized meal plan. The meal plan is designed to help you navigate your way through meals, so you can get started on your way to reaching your goal.
You will also receive a variety of helpful tips and important guidelines to help you navigate tricky situations. You will receive recipes, too.
After the initial session, we schedule weekly or biweekly follow-up sessions, as our schedules permit. Th sessions are used to check in, monitor progress, make adjustments, answer questions, etc.
If for whatever reason you’re unable to make it to your appointment, we ask that you notify us at least 24 hours in advance. That said, however, the more time you can give us, the better we can serve all clients—including you.
Please keep in mind that payment is due upon the first visit (i.e. initial session).
We accept the following methods of payment: credit card, electronic money transfer or cash.
✓ outline how it works in greater detail
✓ discuss your goal(s)
✓ explain how billing and pricing work
✓ address concerns about insurance
