What is pre-diabetes?

Prediabetes is a condition in which a person’s blood sugar levels are elevated, but not to the extent that would diagnose them with type 2 diabetes (T2D).

It is a condition that precedes the diagnosis of T2D if not managed via dietary, lifestyle, holistic and/or medical interventions.

Pre-diabetes diagnosis

In Canada, prediabetes is diagnosed when a person’s hemoglobin A1c (HbA1c) falls within the range of 6.0-6.4% on a blood test. HbA1c is a 3-month average of a person’s blood sugar levels, and is the main lab marker used to diagnose prediabetes and T2D. Fasting blood glucose levels may or may not be elevated when prediabetes is diagnosed.

How can a dietitian and nutritionist help manage prediabetes?

A registered dietitian and nutritionist can help manage prediabetes in the following ways.

 

  • When it comes to the management of prediabetes, the first suggestion one’s health provider is likely to make is regarding lifestyle changes. Typically, it will be encouraged to make changes to diet with the support of a dietitian, exercise and/or weight to help bring the A1c down without the need for medication. Often, repeat bloodwork will be ordered for 3-6 months after adjustments are made to monitor progress and ensure that the A1c has not crept up into T2D territory (A1c of 6.5% or above).
  • The dietary and weight management of prediabetes is where a registered dietitian (RD) can be a very valuable resource. Dietitians can help you make changes to your eating habits to optimize blood sugar and insulin regulation throughout the day. They can also provide sustainable weight loss strategies to help reduce insulin resistance, which further support blood sugar stabilization and insulin production/secretion. These adjustments can help prevent the progression of T2D.
  • The dietary counselling of prediabetes typically involves establishing boundaries/targets around carbohydrate intake–not just the amount of carbohydrates consumed, but also the type of carbohydrates consumed. Protein is also very important for blood sugar regulation, and another area of focus when it comes to the nutrition counselling of prediabetes.
  • A dietitian can also discuss potential supplements that may or may not have a positive influence on the management of prediabetes and reduction of HbA1c.
  • Because diet has a huge impact on blood sugar and insulin levels for people with prediabetes, an assessment with a dietitian is a particularly relevant treatment option.
  • Dietitians can also provide follow-up sessions to monitor progress, address additional concerns, answer questions and make ongoing adjustments to the plan, if necessary.
